Fraud occurs when an individual or group knowingly deceives a person or business for financial gain.


Filing a Fraud Report

  1. Identify the type of fraud you are reporting from the Reportable Incidents list below.
  2. Click the type of fraud to download a fillable PDF form.
  3. Complete the form and use the SAVE AS function to save it to your device. Save the file with your first initial and complete surname (e.g. John Smith will save their report as jsmith.pdf).
  4. After saving the form, use the Report Fraud link below to provide additional details. When you reach the Incident Details section, click the Upload button to share your PDF.
